EVENT WAIVER & RELEASE FORM

This Liability Waiver & Release Form (the “Agreement”) is a written agreement between Pollock Investments Inc., d/b/a Veritiv Pollock (the “Company”) and the individual who is attending (the “Participant”) this Vendor Round Tables and/or VIP Meet and Greet (the “Event”). The Agreement is being entered into in consideration of the benefit of participating in the above referenced Company event. The Participant hereby acknowledges and agrees to the following:


1. Voluntary Participation. Participant understands and confirms that participation in the Event offered by the Company is voluntary. The Participant warrants that he or she is in overall good health and that no bodily or mental condition would create an unreasonable risk of harm to himself or herself in participating in any activities or games at the Event that requires physical or mental exertion.


2. The Assumption of Risk. Participant understands and acknowledges that there may be potential risks involved related to participation in the event. Participant assumes all risks, known and unknown, in any way connected with the Participant’s participation in the event. Participant accepts full responsibility for any liability, injury, loss, damage or death in any way connected with participation in the event. Participant acknowledges that participation in the event is at the Participant’s sole risk. Participant releases the Company and its parents, subsidiaries, affiliates, successors, owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives from any and all liability or claims which may arise from participation in the event.


3. Travel Activities. If the Event involves travel arranged or paid for by the Company, including but not limited to commercial air travel, ground transportation, hotel accommodations, meals, excursions, tours, vendor activities, and other related events (“Travel Activities”), Participant acknowledges that Travel Activities involve inherent risks, including delays, cancellations, lost baggage, injuries, illness, property damage, accidents occurring in transit, and risks associated with staying in hotels or participating in activities at third‑party venues. Participant voluntarily assumes all risks associated with Travel Activities and agrees that the Company is not responsible for the acts, omissions, or negligence of airlines, airports, hotels, transportation providers, tour operators, vendors, restaurants, event partners, or any other third‑party service providers. Participant agrees that the Company makes no representations or warranties regarding the safety, condition, or performance of any third‑party services or facilities. If travel requires government‑issued identification, passport, visa, health documentation, or other entry or travel credentials, Participant is solely responsible for obtaining and maintaining such documentation. The Company is not responsible for any denial of travel or related losses resulting from Participant’s failure to provide proper identification or credentials.


4. Conduct and Property. Participant agrees to conduct himself/herself responsibly and professionally throughout the Event and during all Travel Activities. Participant is responsible for any damage caused by Participant or Participant’s guest to hotel rooms, transportation vehicles, vendor locations, or any other property. Participant agrees to reimburse the Company for any charges, penalties, or losses resulting from such conduct. Participant acknowledges that the Company is not responsible for loss, theft, or damage to Participant’s personal property at any time during the Event or Travel Activities.


5. Medical Treatment and Emergency Authorization. Participant authorizes the Company to secure emergency medical care or transportation if deemed necessary and agrees that the Company assumes no responsibility for the cost or quality of such care. Participant acknowledges and agrees that he or she is solely responsible for any medical expenses incurred as a result of illness, injury, or accident during the Event or Travel Activities.


6. Guest(s). If the Participant brings a guest, the Participant agrees that all terms of this Agreement apply equally to such guest, and Participant assumes full responsibility for ensuring the guest’s compliance with this Agreement. Participant further agrees to indemnify the Company for any claims arising from the actions or conduct of Participant’s guest during the Event or Travel Activities.


7. Photography/Media. Participant grants the Company the irrevocable right to photograph, record, or otherwise capture Participant’s likeness during the Event, and to use such media for promotional, advertising, or other business purposes without compensation.


8. Consumption of Alcohol. Participants twenty-one (21) years of age and older are able to drink alcohol during the event, if applicable, should they choose to do so. However, alcohol consumption is at the Participant’s own discretion, risk, and liability. Excessive alcohol consumption is not condoned by the Company. The Participant hereby assumes all risks involved with the consumption of alcohol and unconditionally holds the Company harmless.


9. Indemnification. Participant agrees to indemnify, defend, and hold harmless the Company and its parents, subsidiaries, affiliates, owners, officers, directors, employees, agents, vendors, partners, insurers, successors, assigns, and any third‑party providers associated with the Event or Travel Activities from any claims, demands, actions, losses, liabilities, or expenses (including attorneys’ fees) arising out of or related to: (a) Participant’s participation in the Event or Travel Activities; (b) Participant’s actions, misconduct, or negligence; or (c) any injury, illness, or damage caused to third parties or property by Participant or Participant’s guest.


10. Modification. The provisions of this Agreement can only be modified by a written instrument executed by an authorized agent of the Company.


11. Entire Agreement. This Agreement constitutes the entire Agreement and understanding between the Participant and the Company, and cancels, terminates and supersedes any prior agreement or understanding relating to the subject matter hereof.


12. Severability. If any part of this Agreement is found to be void by law, the remaining provisions of this Agreement will nevertheless be binding with the same effect as though the void portions were deleted.


13. Governing law. The construction and meaning of the terms and provisions of this Agreement will be interpreted in accordance with the laws of the state of Texas.


14. Binding Effect. Participant acknowledges that this Agreement is binding upon Participant’s heirs and assigns, personal representative, beneficiaries and next of kin.


By signing below, I acknowledge that I have carefully read this Agreement, understand its terms, and voluntarily agree to be bound by it. I understand that I am waiving substantial legal rights, including the right to sue the Company. I agree to release and hold harmless the Company from any and all liability for injuries, damages, or losses occurring during the Event or Travel Activities, including travel to or from the Event.
